RECEIPT OF BIDS: Procurement by competitive proposals are being sought by Cornerstone Family Healthcare. Bids are being requested, on applicable forms and as indicated in bid documents and are to be received by Cornerstone Family Healthcare on or before 07/14/2017 , by 4:00 p.m. (local time). A mandatory pre-bid walkthrough is scheduled for 07/07/2017. The solicitation is in the accordance with the Drawings, Specifications, and other Contract Documents prepared by David Wieboldt, 235 Mountain Road, Port Jarvis, New York 12771 Bids will not be opened publicly. Project is being bid as a single prime project.
The Information for Bidders, Drawings, Specifications and other Contract Documents may be requested through Cornerstone Family Healthcare and are currently available. Addenda, if any, will be issued to only those persons whose name and address are on record of having obtained the Contract Documents.
Contract Documents are available by mail, upon written request to Cornerstone Family Healthcare. Documents will be forwarded via regular mail. Contract documents are also available to be picked up at Cornerstone Family Healthcare, and Corporate administrative offices – 2570 Route 9W, Suite 10, Cornwall, NY. 12518; (845) 220-3100, upon written or telephone request. Bidders requesting documentation will receive a package containing a digital copy of the plans and specifications. The cost of producing drawings and specifications is not reimbursable by the owner.
Each bid shall be accompanied by an acceptable form of Bid Guarantee in an amount equal to at least five (5) percent of the amount of the Bid payable to Cornerstone Family Healthcare as a guarantee that if the Bid is accepted, the Bidder will, within fifteen (15) days after the award of the Contract, execute the Contract and file acceptable Performance and Labor and Material Payment Bonds and Certificate(s) of Insurance.
OWNERS RIGHTS RESERVED: Cornerstone Family Healthcare hereinafter called the Owner, reserves the right to reject any and all Bids and to waive any informality or technicality in any Bid in the interest of the Owner.
No bidder may withdraw his bid within forty-five (45) days after the actual date of the opening thereof. All bid prices to be valid for forty-five (45) days after bid due date.
Subject to the provisions of Article 28, Part III of the New York State Tax Law and the provision of the Contract Document. : In accordance with the regulations within the State Master Contract between Cornerstone Family Healthcare and the New York State Department of Health, the final awarded bidder will be required to fill out a Vendor Responsibility Questionnaire and be bound to the same constraints bestowed upon the awardee by the Grantor. Additionally, the Owner is exempt from payment of sales and compensating use taxes of the State of New York and cities and counties on all materials supplied to the Owner pursuant to this contract.
The Owner hereby notifies all bidders that it will affirmatively insure that in regard to any contract entered into pursuant to this advertisement, women and/or minority-owned business enterprise will be afforded full opportunity to submit bids in response to this invitation and will not be discriminated against on the ground of race, color, national origin in consideration of an award.
