HVCS is a division of Cornerstone Family Healthcare. The two not-for-profits merged in March 2021.

Mission

HVCS’ mission is to promote prevention and wellness for all people, to encourage life-affirming decisions maximizing their quality of life, and to coordinate care for those living with complex health conditions.

Programmatic Summary

Hudson Valley Community Services takes a different approach to well­ness. We  tailor our services to your needs.  We can connect you to every­thing you need to maximize your quality of life.

HVCS covers seven counties: Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster and Westchester.  We have an extensive network of collaborators and partners within each community, yet we are still small enough to stay connected to you. We are part of the three health homes  which cover our area. We have over thirty years of experience connecting low-income, marginalized communities to the healthcare and social supports you need to reach your full potential.

HVCS offers an expansive range of healthcare coordination, support services and health education programs that help you access and stay connected to all the services and information you need.  We specialize in assisting people living with chronic diseases, including HIV/AIDS, substance abuse, behavioral health issues, diabetes, hepatitis C, heart disease, obesity, COPD, Alzheimer’s, depression, and many more through our Medicaid-reimbursed care management program.

We started as an HIV/AIDS organization and still offer support, resources and services for HIV-positive people.  Whatever you need outside of a medical office, we can help. HVCS served approximately 3,700 clients in 2020.

We also have prevention education programs to reduce the spread of HIV, sexually transmitted infections (STIs) and Hepatitis C. We also believe in harm reduction and offer assistance for people with substance use disorder. And this is only a quick summary of what we offer.
