This notice is issued in compliance with the Facility Closure Plan Guidelines outlined in DHDTC DAL#: 23-06 and other applicable state and federal regulations.

The purpose of the public hearing is to:

- Provide information regarding the proposed closure of the Cornerstone Family Healthcare facility at 290 Broadway and transfer of patient care to 100 Broadway.
- Discuss the impact on the community, including access to healthcare services.
- Gather input and address concerns from stakeholders, including residents, patients, and employees.
Representatives from Cornerstone Family Healthcare will be present to respond to questions and provide additional details regarding the closure plan.
