The Highest Level Patient-Centered Medical Home

Newburgh, NY (December, 21st  2018) – Cornerstone Family Healthcare is proud to announce it has once again been awarded recognition by The National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH).  The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.   It recognizes physician practices prioritizing the physician-patient relationship, coordinating care for patients across multiple settings, and engaging in a team approach to improve patient care.

Our medical home program has excelled at improving access to care, communication, and the quality of care patients receive.  According to Dr. Avi Silber, Chief Medical Officer of Cornerstone Family Healthcare, “We have utilized our electronic health records to track test results, follow patient referrals, identify patients in need of follow up care for ongoing medical problems, and to reach out to patients who would benefit from more aggressive health care management.”

 

Cornerstone provides more than 45,000 adults and children in our region with their comprehensive health care.  Last year, we provided primary and preventative health care services totaling more than 250,000 patient visits. Our staff of more than 560 employees includes over 50 Board Certified/Board Eligible Physicians, and Dentists, physician’s assistants, nurse practitioners, a large nursing staff, phlebotomists and laboratory professionals, and social work and counseling staff.
